Quick question - would River call anyone other than the Doctor "Sweetie"? In particular, would she say it sarcastically, as in e.g. "Sweetie, you've been a naughty girl."

As a verbal affectation in forms of address, the use of sweetie does tend to apply to multiple people - and the intent of its use in context would be conveyed by tone of voice in real life. However, River Song *might* be unusually reserving it for just one person. To me, your example seems slightly off (eg I'd put sweetie at the end not the start to make the sarcasm clearer).
